Robust Warranty
Every appliance from Five Star Orthodontic Laboratory & Supply is custom-made, tailored to the specifications outlined by licensed dentists. Our products come with a solid 60-day warranty against any workmanship or material defects. Should any of our products demonstrate such flaws, we commit to either repair or replace them. However, damages due to accidents, neglect, or misuse fall outside this warranty’s purview.

Complete an Rx form and send in your impressions, models, or intraoral scans. Make sure to check the “Call Doctor” box in the top right corner if you desire a consultation before fabrication. You can easily download our Rx form here.

If sending impressions, ensure they are taken with Kromopan, Tropicalgin, or PVS. Wrap them in a wet paper towel (to minimize the chance of shrinkage) before packing them into the shipping box.
If sending models, write the patient’s name and doctor’s name on the models. Wrap them individually with bubble wrap before packing them into the shipping box. If you have an intraoral scanner, upload your digital impressions and Rx to our online portal.
*Refer to the “Appliances” section of our website for material requirements for each appliance. Some appliances require specific materials to be sent to the lab.
Use Five Star’s pre-printed UPS 2nd Day Air labels for shipping, and Five Star will cover the inbound shipping costs. If you’re in the DFW Metroplex, give us a call, and we’ll pick up your case. Access our pre-printed UPS 2nd Day Air labels here.
If the “Call Doctor” box is checked on the Rx form, Five Star will contact the Doctor to discuss the case and method of treatment at no extra charge. If a consultation isn’t required, the case goes directly into fabrication upon receipt.
An invoice accompanies the case at the time of shipment. Additionally, Five Star sends each customer a monthly statement.
First-time customers are requested to pay with a credit card or COD upon shipping. After establishing a continuous business relationship, customers can settle via the monthly statement on a net 30-day basis using a check or credit card. Auto-pay is available upon request, with the card charged on the 1st of every month.
