How do I place an order? Expand

Complete an Rx form and send in your impressions, models, or intraoral scans. Make sure to check the “Call Doctor” box in the top right corner should you desire a consultation before fabrication. You can easily download our Rx form here.

How do I set up an account with Five Star? Expand

To establish an account, please complete the New Account Form and email it to beth@fivestarortho.com.

What do I need to send in for an appliance to be made? Expand

If sending impressions, please make sure it was taken with Kromopan, Tropicalgin or PVS and wrap them in a wet paper towel (to minimize the chance of shrinkage) before packing into the shipping box.

  • If sending models, please make sure to write the patient’s name and doctor’s name on the models and wrap them individually with bubble wrap before packing into the shipping box.
  • If you have an intraoral scanner, simply upload your digital impressions and Rx to our online portal.

How do I ship my case to Five Star? Expand

Please send your case using Five Star’s pre-printed UPS 2nd Day Air labels and Five Star will pay for inbound shipping. If you are located in the DFW Metroplex, give us a call and we’ll come pick up your case. You can access our pre-printed UPS 2nd Day Air labels here.

What happens when my case gets to the lab? Expand

If the “Call Doctor” box was checked on the Rx form, Five Star will contact the Doctor to discuss the case and method of treatment at no charge. Should consultation not be necessary, the case goes straight into fabrication upon receipt.

Does Five Star send an invoice with each appliance? Expand

Five Star will include an invoice with the case at time of shipment. In addition, Five Star sends each customer a monthly statement.

How do I pay for my appliances? Expand

Five Star requests first time customers to pay with credit card or COD at the time of shipping. Once an ongoing business relationship is established, customers can pay via the monthly statement on a net 30-day basis using check or credit card. Auto-pay is available upon request (card is charged on the 1st of the month).
