How do I set up an account with Five Star?
To establish an account, please complete the New Account Form and email it to email@example.com.
What do I need to send in for an appliance to be made?
If sending impressions, please make sure it was taken with Kromopan, Tropicalgin or PVS and wrap them in a wet paper towel (to minimize the chance of shrinkage) before packing into the shipping box.
- If sending models, please make sure to write the patient’s name and doctor’s name on the models and wrap them individually with bubble wrap before packing into the shipping box.
- If you have an intraoral scanner, simply upload your digital impressions and Rx to our online portal.

How do I ship my case to Five Star?
Please send your case using Five Star’s pre-printed UPS 2nd Day Air labels and Five Star will pay for inbound shipping. If you are located in the DFW Metroplex, give us a call and we’ll come pick up your case. You can access our pre-printed UPS 2nd Day Air labels here.
What happens when my case gets to the lab?
If the “Call Doctor” box was checked on the Rx form, Five Star will contact the Doctor to discuss the case and method of treatment at no charge. Should consultation not be necessary, the case goes straight into fabrication upon receipt.
What is Five Star’s turnaround time?
Five Star will fabricate the appliance within 5 business days upon receipt or design approval and ship back to the Doctor via UPS ground, 2nd Day, or Overnight per the Doctor’s request. (Doctor pays outbound shipping). Appliance fabrication can be rushed for an additional fee.
Does Five Star send an invoice with each appliance?
Five Star will include an invoice with the case at time of shipment. In addition, Five Star sends each customer a monthly statement.
How do I pay for my appliances?
Five Star requests first time customers to pay with credit card or COD at the time of shipping. Once an ongoing business relationship is established, customers can pay via the monthly statement on a net 30-day basis using check or credit card. Auto-pay is available upon request (card is charged on the 1st of the month).
